I understand that participating in “Cannonballs for Charis” is a potentially hazardous activity. I should not enter and participate unless I am medically able, properly attired, know how to swim, and am otherwise prepared. I assume all risks associated with participating. I understand that event officials may terminate my participation in the contest at any time, for any reason, and I agree to abide by all decisions of event officials. Having read this waiver and these facts and in consideration of your accepting my registration form, I, for myself and anyone entitled to act in my behalf, waive and release any and all persons and entities connected with Charis Health Center, including event officials, volunteers and all sponsors, their representatives and successors, from all claims or liabilities of any kind arising out of my participation in this event, even though that liability may arise out of negligence or carelessness on the part of the persons named in this Waiver. I grant permission for the administration of medical aid in case of emergency. I also grant permission to all of the foregoing to use any photographs, motion pictures, recordings, or any other record of this event for any legitimate purpose without compensation to me.
